It really isn't as bad as it sounds-though it is incredibly annoying.
You should be prepared to just fight the entire world-or rush your targets down and quickly end the war. There's a cap as to the total amount of threat you can get per county, though it's pretty meaningless by the time you're an Empire, because no matter what, even a little bit of expansion is going to kick you through the roof. Rule of the thumb is that the bigger you are, the faster you gain threat.
